Headlines like “the year of the beautiful nose” are easy to write and hard to check.
Key takeaways
- ISAPS counts rhinoplasty at about 1 million procedures a year worldwide, second only to eyelid surgery among face and head operations; its 2025 release shows no sharp surge.
- The clearest shift is in taste: AAFPRS surgeons report patients asking for natural-looking, subtle results.
- Preservation rhinoplasty looks comparable to traditional hump reduction in selected patients, but it is not proven better and does not suit every nose.
- AI-generated 'perfect' noses ignore bone, skin and airflow, and photos of real people can be brought to a consultation instead.
Is rhinoplasty really booming in 2026?
Not in the way the headlines suggest. The International Society of Aesthetic Plastic Surgery (ISAPS) published its 2025 global survey on 29 September 2026. It lists rhinoplasty at “over 1 million” procedures, the second-largest face and head procedure after eyelid surgery, and says 57% of rhinoplasties were on patients aged 18 to 34. The release gives no year-on-year change for rhinoplasty. The year before, ISAPS reported about 1 million rhinoplasties and a 10% decrease.
So the safest reading is a large, steady procedure, not an explosion. These surveys count procedures reported by plastic surgeons who responded, so they describe the profession’s activity, not everyone who had a nose job.
What do patients mean by a “natural” nose?
The shift that surgeons themselves describe is taste. The American Academy of Facial Plastic and Reconstructive Surgery (AAFPRS) released its 2025 survey in February 2026. It says patients are seeking natural-looking, thoughtfully planned approaches, and that rhinoplasty, facelifts and eyelid surgery remain the top surgical procedures for both women and men. The release does not say how many members were surveyed, so treat it as a trend signal, not a measurement. For men specifically, see facial plastic surgery for men.
In practice, “natural” usually means a nose that fits the face and your own features, rather than a copy of a template. Our guide to natural nose rhinoplasty explains the goals and trade-offs in more detail.
Does preservation rhinoplasty deliver?
Preservation techniques keep more of the nose’s original structure, typically on the bridge, instead of removing a hump and rebuilding. The evidence is growing but is not a clear win:
- A 2023 systematic review found that most studies were low-level and rarely used validated patient-reported measures, even though results were consistently positive.
- A 2026 systematic review of 20 studies found preservation and structural hump reduction equivalent in aesthetic and functional results at 6 to 12 months in appropriately selected patients, with pooled revision rates typically 2 to 4%.
- A 2026 meta-analysis of airway outcomes of 22 studies (2,058 patients) found comparable functional results between the two approaches.
- A 2025 meta-analysis of four randomized trials (419 patients) did favor preservation, so the size of any advantage is still debated.
Taken together, preservation looks at least as good as traditional reduction for suitable noses, and not every nose is suitable. Whether you are a candidate is a surgeon’s judgment after examining your anatomy.
Do new tools change how predictable results are?
Two tools get most of the attention. Piezoelectric (ultrasonic) bone cutting has a decent trial record for the early recovery period. A 2026 meta-analysis of 19 randomized trials found less swelling, bruising and pain in the first week and less mucosal injury than with conventional instruments, while the authors note that time-based effects remain uncertain. That supports “easier early recovery” for some patients, not a proven better final result. See our piezo rhinoplasty guide and, for the rotating power tool some clinics advertise, our guide to burr rhinoplasty.
AI imaging is more double-edged. In a September 2026 article, the American Society of Plastic Surgeons warns that AI-generated noses ignore bone structure, skin thickness and airflow, while surgical simulation built from a 3D scan of your face can account for real proportions. Read how that works in our guide to simulation before a nose job.
What about claims that one country leads?
Several marketing pages call a country or a group of surgeons the world leader in preservation or technology. No published registry or survey ranks surgeons by technique, so that claim cannot be checked. A fairer test is the individual surgeon’s own record. Our guide on finding your rhinoplasty surgeon lists what to ask.
Questions to ask your surgeon
- Which approach do you recommend for my nose, and why this one rather than the alternatives?
- Am I a candidate for preservation, and what happens if you need to switch techniques during surgery?
- How many noses like mine have you operated on, and can I see unedited, dated results?
- Can you show a simulation based on my own face, and how closely do your results usually match it?
- How are airflow and breathing protected in this plan?
